Service Program Authorization Information

A list of EMS Service Programs with recent status changes, a form for changing service program status, and a template for EMS Personnel Files can be downloaded here.

All Iowa ambulance services must have an EMS contigency plan that will be put into operation when 24/7 coverage is not possible due to unforseen circumstances. This file contains a sample form that can be modified by service programs to meet their individual needs.

Ambulance services seeking to provide nontransport coverage in addition to ambulance authorization must provide a transport agreement. This file contains a sample form that can be modified by service programs to meet their individual needs.

Iowa law permits the use of white flashing lights for identification of emergency medical care providers and service medical directors.
