Service Program Authorization Information

All Iowa ambulance services must have an EMS contigency plan that will be put into operation when 24/7 coverage is not possible due to unforseen circumstances. This file contains a sample form that can be modified by service programs to meet their individual needs.

EMS Contingency Plan

Ambulance services seeking to provide nontransport coverage in addition to ambulance authorization must provide a transport agreement. This file contains a sample form that can be modified by service programs to meet their individual needs.

Transportation Agreement for Ambulance Service Programs

For staffing purposes, all Iowa nontransport service programs must have a transport agreement. This file contains a sample form that can be modified by service programs to meet their individual needs.

Iowa law permits the use of white flashing lights for identification of emergency medical care providers and service medical directors.
