When the application for a Funeral Home Establishment is submitted because there is a change in ownership of the Establishment, please submit the License Certificate and the License Card with the expiration date.

• Select "License Search".
• Insert the licensee's name or license number.
• Select the profession from the dropdown list.
• Select "Print" for a paper copy.
