Physician assistants shall use the below form to notify the board of the identity of their supervising physicians at time of license reactivation. Mail the Report of Supervising Physicians with the reactivation form.

You may now verify and print the licensure status of professionals and establishments by following five easy steps.

• Go to www.licensediniowa.gov
• Select "License Search".
• Insert the licensee's name or license number.
• Select the profession from the dropdown list.
• Select "Print" for a paper copy.
