If the license has been inactive for two or more years, the licensee shall be required to complete a mandatory criminal background check. To reactivate your license, contact the board office at 515/281-4287 and the reactivation packet will be sent to you.

If the license has been inactive for less than two years complete the reactivation form above.

Once licensed you will be able to view and print your licensure status by following these five easy steps. The board office will mail a license certificate and wallet card to you via regular mail.

• Go to www.licensediniowa.gov
• Select "License Search".
• Insert the licensee's name or license number.
• Select the profession from the dropdown list.
• Select "Print" for a paper copy.
