Any facility that will provide mammography services in the United States must be accredited by an FDA approved accrediting body. Iowa is approved to accredit all facilities located within the State of Iowa borders.
If your facility accredits with the State of Iowa, authorization is included in the accreditation process.
If your facility uses the ACR as your accrediting body you must complete the State of Iowa Authorization process in addition to any ACR requirements.

Existing accredited facilities will complete this for every new unit installation prior to use of the new unit.

Existing State of Iowa accredited facilities will complete this once every three years.

This attestation can be signed by the lead interpreting physician and submitted for reaccreditation - instead of sending in all of the personnel documentation. For more information see FAQ.
Any facility that will provide mammography services in the State of Iowa must be authorized by the IDPH mammography program. This applies to all fixed facilities within Iowa as well as to facilities located outside of Iowa who provide mobile mammography services within State of Iowa borders.
