Medical Physicists performing surveys of mammography and/or stereotactic breast biopsy units in Iowa must be approved by the Bureau of Radiological Health prior to performing any surveys for Iowa mammography facilities.
Medical Physicists must register with the Bureau of Radiological Health and submit the appropriate applications and documentation to prove that the required qualifications for performing mammography and stereotactic breast biopsy surveys and consultations are met.

The above registration form and applications are for medical physicists who will be performing surveys of mammography facilities and equipment. If you are a service company or a medical physicist who will be providing only shielding calculations or surveying units other than mammography or therapy units then you must complete the “Servicing and Services Registration Form” found on the Radiation Machines page.
