The Master Index Card details a patient's complete vaccination history on a two page record.

Should be used as an internal document for the clinic.

If current clinic administered the vaccination the client/parent/guardian signature line will remain blank.

This record can be printed and placed in a patient's chart to document the immunization history including the patient's name, demographic information, VFC eligibility, vaccine types, vaccination dates, manufacturer and lot number, route and site, and VIS date.
