The Physician-Patient List can be printed to report to a local Primary Care Physician or to create a report for a physician in an office.

The list will include all of the Physician's patients who were immunized by your office during the time period that is selected.

The name of the patient, their date of birth, vaccine type, and vaccination date are included in this list.
