This report should be reviewed prior tot recall notice, made to verify vaccinations needed.

If found vaccinations truly not needed the clinic needs to verify schedule setup and dates administered.

The reminder list can be used to conduct recall activities.

The reminder list includes the patient's name, date of birth, parent or guardian name, address, primary phone number, and a list of the vaccines that the patient is due for.

Due to the database intensive nature of this report, it may not be immediately available for printing.
