To apply for the reactivation of an inactive salon license complete the following form and return it to the board office.

Once your license has been reactivated the board office will mail a license card to you via regular mail. You can also view and print your licensure status by following these five easy steps.

• Go to www.licensediniowa.gov
• Select License Search.
• Insert the licensee's name or license number.
• Select the profession from the dropdown list.
• Select Print for a paper copy.
