School Outbreak Reporting System

Guidelines for reporting:

• The Iowa Department of Public Health requests reports of school absence due to illness that meets of exceeds 10% of a school's enrollment.
• Total absent and enrolled are needed for each day absence due to illness ≥10%. Single day reports or an entire week's worth of reports may be entered at one time.
• Reports should be submitted by building, not district.
• Reports should count only student absence, not staff or volunteer.
• The school name and county are published in the Iowa Influenza Surveillance Network weekly report. The report may be found at http://www.idph.state.ia.us/adper/iisn.asp.
• Thank you for taking time to report!

You will need the following information to access the reporting system.

• District code
• Building code

Finding your district and building code - These codes are used during your immunization and dental record audits. The codes by school may also be found on the Iowa Department of Education's website. Choose the 2011-2012 Educational Directory and open the file.
