Model Business Policy Toolkit (Linn County)

This toolkit, created by Linn County Public Health, includes a lot of information about businesses creating a tobacco-free policy. It also includes a sample model policy. Because of it's size it has been broken into four pdf files.

Quitline

Quitline Iowa

Below are the four different fax forms that can be used by medical providers to refer their patients to Quitline Iowa.

The standard fax form for telephone counseling, A Medicaid Prior Authorization fax form for Nicotine Replacement Therapy (NRT) like patches, gum and lozenges, A Medicaid Prior Authorization fax form for Chantix(tm) and Bupropion.

A fax form for Women Infants and Children (WIC) clinics is also available. The WIC form is the same as the standard form; no separate prescription benefit is available through WIC. Use of this form allows IDPH to better track those fax referrals sent by WIC Clinics.

The following counties are considered 'metro' for purposes of this assessment:

Blackhawk, Cerro Gordo, Dubuque, Johnson, Linn, Polk, Pottawattamie, Scott, Story, Woodbury.

Please remember that retailer assessments need to be entered by June 15, 2012.

School Policy

Model School Policy

This link will open a document with language for a model "tobacco free campus" policy. This document was updated in March, 2012 to include nicotine products. If a school in your area has a comprehensive policy for "tobacco" but does not specifically include nicotine products it is still considered a comprehensive policy and that school does not need to change their policy in order to receive signs. If a school is updating their old policy, we suggest that they use this model language.
